ABSTRACT

Successful nailing requires fracture reduction before and control of reduction during nail insertion. Closed or limited open fracture reduction is achieved with the help of the fracture-table, large distractor, external fixator, pointed reduction forceps, K-wires, lag screws, cerclage wires or a small plate. A combination of reduction tools may be used. For uncomplicated nail insertion, the choice of the correct entry portal is indispensable. The use of a guide wire is recommended for nailing of metaphyseal fractures. Intramedullary blocking (poller) screws prevent or correct malalignment and add to the overall stability. Optimal position of the nail, good fragment reduction with narrow fracture gaps and adequate stability are the most important prerequisites for uneventful fracture healing. Fracture pattern, fracture localization and the design of the selected nail ask for specific reduction and insertion techniques, which have to be chosen before surgery. More... »
