ABSTRACT

Cancers of the gynecological tract are diagnosed based on histopathological examination, and further subclassified using the combination of tumor cell type and site of origin. Thus, an endometrioid carcinoma, composed of cells resembling those of normal endometrial glandular epithelium, or a serous carcinoma, composed of cells resembling those of normal fallopian tube epithelium, can arise from ovary, fallopian tube, endometrium or cervix. With the advent of genomics it is clear that this classification system reflects underlying genetic alterations in the different tumor types. For example, tumors of endometrioid and serous type have profoundly different mutation profiles. Furthermore, tumors of similar cell type (e.g. endometrioid) arising from ovary versus endometrium also show genetic differences, albeit much more subtle, when compared to the differences seen between tumors of different cell types. There will be increasing movement towards more individualized therapy for patients with gynecologic cancers, this will undoubtedly reflect the underlying molecular abnormalities in the different tumors, and this trend has already started with the emergence of subtype specific management of gynecological cancers; there are clinical trials ongoing for clear cell carcinoma and mucinous carcinoma of ovary, for example, which in the past were treated in an identical fashion to other ovarian cancer subtypes.
